Clinical Observations of Bacterial Pneumonia in Adults

Abstract
A clinical study was made on 155 cases of pneumonia hospitalized at National Medical Center from Jan., 1971 to Aug., 1976.
The results were as follows
1. The incidence of pneumonia among all the hospitalized patients in department of Internal Medicine, during the same period was 1. 7% and male to female ratio of the disease was 1. 2 : 1.
There was male preponderance in the pneumococcal and Klebsiella pneumonia.
Peak incidence of age was in the third decade.
2. The causative organisms were proved in 92 cases out of 155(59.4%) by sputum and blood culture.
Staphylococci was isolated most commonly in 25% of patients. .
3. Fever was most common symptom in this study and cough, dyspnea, chillness, chest pain and blood-tinged sputum followed in order of frequency.
4. Anemia (below Hgb 10 gm%) was observed in 18% of patients. Leukocytosis appeared in 51% and leukopenia in 9.7%of patients. ESR was markedly increased in Klebsiella and Staphylococcal pneumonia.
5. Average duration of fever was 7.6 days in all patients. Defervescence appeared within 5 days in 60%.
The average durations of fever in Klebsiella and staphylococcal pneumonia were longer than of pneumococcal pneumonia.
6. Associated diseases were found in 81. 3% of patients and disturbance of consciousness was most common condition.
7. Frequency of complications was 34. 2% and pleural effusion was most common (23.2%).
8. 23 cases of patients were died and the mortality rate was 14.8%.
